What is a Care Plan?

Care Plans are designed to optimise the health of people who live with a chronic condition/s. These are also known as GP Management Plans and Team Care Arrangement and were previously called Enhanced Primary Care (EPC) items.

Who is eligible for a Care Plan?

Medicare classifies a chronic condition as a condition that has, or is likely to, last longer than 6 months.

Some common examples are, but not limited to:

  • Diabetes
  • Asthma
  • chronic kidney disease
What is involved in a Care Plan appointment?

You will have an appointment with both the nurse and your usual GP on the same day. First you will see one of our expert nurses who will work with you to outline your healthcare goals and identify members of your treating team that will help you achieve these. The nurse will write the plan with you, and identify any areas needing clarification with your GP. And then you will follow up with your GP to discuss any further additions to your plan, order any tests needed, and sign the allied health referrals.

Health checks

What is a health check?

Health Checks, also known as health assessments, are designed to support your physical, social and emotional wellbeing. A nurse can provide health education, prevent future illness such as advising on available immunisations, and pick up early treatable issues such as high blood pressure.

How much does a health check cost?

Your health check will be bulk billed.

Who is eligible for a governenment-funded health check?

45-49 year olds: These are available as a once off health check to any person aged 45-49 years. This is anopportunity to have an assessment of your current health status. We will alsoconsider your future health risks and discuss strategies to prevent these.

75+ years: These are available as a once off health check to any person aged 45-49 years. This is anopportunity to have an assessment of your current health status. We will alsoconsider your future health risks and discuss strategies to prevent these.

Aboriginal & Torres Strait Islanders: These health checks are available to any person who identifies as Aboriginal or Torres Strait Islander of any age and are available annually. The nurse records information about your health, such as your blood pressure, blood sugar levels, height, and weight. You might also have a blood test or urine test. It is also an opportunity to talk about the health of your family.

Holter & blood pressure monitoring

What is a holter monitor?

A Holter monitor is a type of portable electrocardiogram (ECG) that records the heart’s rhythm for 24 hours to detect any abnormalities with your heartbeats.

What is a 24-hour blood pressure monitor?

A 24-Hour Blood Pressure monitor is a portable device attached to a BP cuff that records your blood pressure regularly over 24 hours to determine if one is at risk of hypertension.

You will need an initial appointment with a doctor to determine if you are suitable for a Holter monitor or a 24 Hr BP monitor.

Call our reception team on 03 9485 7700 to book an appointment. Once the GP orders these tests, you will need to book an appointment with one of our nurses to set up the monitor, and again the next day to take it off.

The GP will receive the result within a week, and generally results would be given to you at an appointment.

What is wounds care?

The nursing team at Northside Clinic provide wound care and management for:

Ear irrigation

What is ear irrigation?

Ear irrigation is a procedure that uses water to remove excess ear wax from the ears.

You will need an initial appointment with a doctor to determine if you require an ear irrigation procedure at the clinic.

Iron infusions

An iron infusion is a treatment option for people aged 14 years or older with iron deficiency anaemia who require rapid iron repletion but are unable to take, or fail to respond to, oral iron preparations. You will need to discuss whether you are suitable for iron infusion therapy with your GP prior to making an appointment for an iron infusion.

Immunisations available at Northside Clinic include:

  • Childhood immunisations, including catch-up schedules
  • Adult immunisations as per the National Immunisation Schedule
  • Seasonal Influenza immunisations
  • Covid vaccines
  • Mpox vaccines

Procedures available at Northside Clinic include:

  • Suturing & removal of sutures
  • Removal of lesions
  • Skin biopsy
  • Hormone implant insertion
  • Toenail resection
  • Treatment of undisplaced fractures

There is a fee involved with all procedures at Northside Clinic. Please discuss with your treating GP or nurse.

You will need to make an initial appointment with your doctor to discuss your procedure and what to expect. Following this appointment, a Northside Clinic staff member will contact you to arrange a time for your procedure.

Contraception

Contraceptive procedures available at Northside Clinic include:

  • Intra Uterine Device insertion and removal
  • Diaphragm fitting
  • Implanon insertion and removal

There is a fee involved with these procedures at Northside Clinic. Please discuss with your treating GP or nurse.

You will need to make an initial appointment with your doctor to discuss your procedure and what to expect. Following this appointment, you will need to arrange a time for your procedure.

Travel vaccinations

If you are travelling overseas please speak to your GP about travel vaccination to protect yourself against diseases you may encounter. Some countries may require proof of immunisation for certain infectious diseases before you can legally enter that country.

Be mindful that you may need to start some vaccination courses requiring multiple doses 6 to 12 weeks before you leave Australia. Additionally we offer:

  • Altitude sickness prophylaxis
  • Traveller’s diarrhoea treatment
  • IWe are Yellow Fever Accredited travel clinic
