Before filling out this expression of interest form, please be mindful of the following things.  We will try and call you in the next 24-48 hours to book your appointment, our phone number is 03 9485 7700, please save it into
your phone so you know it’s us calling.  

We make two attempts to call and then it is up to you.  If you don’t return our call, we assume you are no
longer interested in booking an appointment.  There is a deposit required to be paid at the time of making your appointment, which is $112.20.  This doesn’t necessarily cover the full cost of the appointment, however you will receive a Medicare rebate if eligible. 

The deposit may also be used to cover the Dr’s time if you decide not to turn up without letting us know and receive a Did Not Attend fee.  If of course, you have a change of mind and let us know at least 2 hours before your appointment, we are happy to refund your deposit.

It is an expectation that as a patient of Northside Clinic, you treat our staff with dignity and respect at all times. That includes in person, over the phone and via other electronic communications (email). We have a zero tolerance policy regarding aggressive and abusive behaviour and you will be asked to leave the clinic and move your records elsewhere if you breach this policy.
