HOW TO SEE A DOCTOR
Northside Clinic sees patients on an appointment basis. To book, call reception on (03) 9485 7700 and our reception staff will arrange an appointment with the doctor of your choice. If your problem is urgent, please inform reception staff so they can arrange an appointment as soon as possible. Appointments are set for 15 minutes, however longer consultations are available upon request. If you have complex, or a number of health issues to discuss with your doctor, please consider booking a longer appointment so that all of your concerns can be investigated. Registered patients of the clinic will receive SMS reminders of their appointments. This is sent out the afternoon before the appointment. Please respond to the message to indicate your confirmation of attendance. If you do not wish to receive reminder SMS messages for appointments, please advise the reception staff. Patients who fail to attend appointments may be charged a non-attendance fee.
Patients using wheelchairs or prams can gain access to the General Practice building using the side gate. Follow the path and ring the doorbell, our staff will let you in. Wheelchair access to the Allied Health clinic is via the back door. There is a lane behind St Georges Road (off Clauscen St) that will give you access to the rear of the building, ring the doorbell and we will let you in.
Medical Records and Your Privacy
Records will be kept of your visits and treatment. Your medical record is a confidential document. It is the policy of this clinic to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of the Nursing, GP and Allied Health staff. All staff and health professionals that work at Northside Clinic and Northside Allied are bound by confidentiality agreements. We comply with the Privacy Act 2000 and the Victorian State Act 2001 including the way we store, use and disclose health information.
The contents of your health records will be divulged only with your consent or where justified by law. You are entitled to see or correct your medical records at any stage. It may be necessary for parts of your medical record to be disclosed to other medical professionals who are providing you with treatment, or tests or during activities necessary to operate our clinic (i.e to our insurer or third parties paying for your treatment)
TRANSFER YOUR MEDICAL RECORDS
If would like your existing medical records transferred from another clinic you will need to provide them with a written request. You can either send them a letter requesting your records be forwarded to Northside Clinic or we can give you a form to complete and fax it for you. Please note that some medical centres charge an administration fee for transferring records.
